当院の看板犬のジュジュが癌の摘出手術をしました。漢方薬治療専門で治療している私が、いざ家族に癌が見つかったとき場合、どう治療を選択するのか、皆様の参考になればと思います。
癌で生活の質QOLを維持するには、炎症のコントロールが大切です。体質改善と合わせて炎症をコントロールできれば、たとえ進行を止められなくても最期まで穏やかに過ごしてくれることが多いです。
当院では基本的には体質と免疫を整えることで炎症をコントロールします。しかし癌の炎症が強くて抑えきれない場合は、ほんの少しの量ステロイドを併用することもあります。ステロイドを使うだけでは、免疫が下がり癌がさらに活性化させるリスクがありますが、漢方薬治療で必要なステロイド量を抑え、免疫の低下をしっかりカバーできれば十分メリットを享受できます。ジュジュの場合も、癌の炎症が強く、漢方薬治療に加え、少しだけステロイド（0.07m/kg;通常の14分の1）を使ってコントロールしました。
癌からの炎症と、腎不全で体調が悪かったジュジュでしたが、漢方薬治療ですっかり元気を取り戻し、おしっこも見た目はきれいな状態にまで戻りました。一ヶ月後のエコー検査でも、少なくとも進行は抑えられたようで、癌の大きさはなんとか2mmだけ縮んでくれたようです。腎臓数値も無事回復してくれました。漢方薬が効いてくれてほっとした気持ちと、やはり移行上皮癌は手強いなというのが正直な気持ちです（たくさん漢方薬を飲みましたから）。
少なくとも今の状態では漢方薬治療で癌の進行を抑えられていると考えられるので、手術後もこのまま進行を抑えることができれば再発も防げる可能性があります。手術の方法は大きく分けて、2通りです。膀胱を全部撮ってしまう全摘術、癌がある膀胱の一部だけ摘出する部分切除です。
膀胱の全摘出が一番再発を防げる方法になりますが、ジュジュの場合は（我々の治療方針では）、生活の質の確保が最優先なので部分切除をすることにしました。手術だけの場合をいうと、根治を目指してというよりは、QOL維持のための術式になります。部分切除の場合、膀胱が小さくなるので、術後は頻尿になりますが、次第に膀胱がまた大きくなり頻尿も数週間で落ちつくようです。ただ、これだけ大きい癌なので、部分切除も摘出も一筋縄ではいかなかったようです、、、

Our mascot dog, Juju, underwent surgery to remove the cancer.I am a specialist in Chinese herbal medicine treatment. I hope it will be helpful to everyone as to how to choose treatment when cancer is found in my family.
Controlling inflammation is important to maintain quality of life QOL at treatment of cancer. As long as we can control the inflammation, it will often stay calm until the end, even if you can't stop the cancer progressing .At our hospital, we basically control inflammation by improving body constitution and adjusting immunity. However, if the inflammation of the cancer is too strong to control, a small amount of steroids may be used in combination. However, there is a risk that immunity will be weakened and cancer will be activated just by using steroids, but if you reduce the amount of steroids required and prevent the weakening of immunity with of Chinese herbal medicine, you can receive the benefits.
In the case of Juju, because the inflammation of the cancer was strong, it was controlled with a small amount of steroid (0.07 m / kg; 1/14 of the usual amount)in addition to the Chinese herbal medicine treatment.
Juju was sick due to inflammation from cancer and renal failure, but he was completely rejuvenated by Chinese herbal medicine treatment, and his pee returned to a clean state .Echo examination one month later showed that at least the progression was suppressed, and the size of the cancer managed to shrink by 2 mm. The kidney function also recovered .I was relieved that the Chinese medicine has worked, and I honestly feel that transitional cell carcinoma is tough (because he took a lot of Chinese medicine).
At least in the current state, it is thought that the progression of cancer is suppressed by Chinese herbal medicine treatment, so if the progression can be suppressed as it is after surgery, recurrence may be prevented.There are two main methods of surgery.Total resection that takes the entire bladder,A partial resection that removes only part of the bladder that has cancer.
Total removal of the bladder is the best way to prevent recurrence, but in the case of Juju (in our treatment policy), quality of life is our top priority, so we decided to do partial resection. Rather than aiming for a cure, it is a technique for maintaining QOL.In the case of partial resection, the bladder becomes smaller, so frequent urination occurs after surgery, but the bladder gradually grows larger and the frequent urination seems to subside within a few weeks.
